Patricia J. Prokopiak passed away on Thursday, September, 20 2018 at Robert Wood Johnson University Hospital at Rahway. She was 79 years old.

Born in Nanticoke, PA, she has resided in of the Avenel section of Woodbridge Township since 1970.

Mrs. Prokopiak was employed with Rahway Hospital for 31 years before her retirement in 2006; and was a communicant of Saint Andrew Catholic Church in Avenel.

She was preceded in death by her husband, Walter S. Prokopiak, Jr., in 2007; great grandson, Shawn; and brothers, Joseph, Francis and Anthony Garnett.

Surviving are her children, Walter S. Prokopiak III and his wife, Ulana, of Colonia, Patrick J. Prokopiak and his companion, Kelly Cawley, of Avenel, Deborah L. Prokopiak-Gomez and her husband, Frank, of South Plainfield and Joseph P. Prokopiak and his wife, Rebecca, of Toms River; sister, Marlene Kupsch of Sayreville; grandchildren, Patrick, Michael, Richard, Heather, Thomas, Alyssa, Franchesca and Joseph; and great grandchildren, Isabella, Julian and Daniel.

Visitation for family and friends will be on Monday, September 24, 2018 from 3:00 p.m. until 7:00 p.m. at Costello-Koyen Funeral Home ( www.costello-koyen.com ) 399 Avenel Street, Avenel. Funeral services will be on Tuesday morning, beginning at 8:45 a.m. from the funeral home, followed at 9:30 a.m. with a Mass of Resurrection at Saint Andrew Catholic Church. Entombment will be in Woodbridge Memorial Gardens in Woodbridge.
